CANCELLATION AND RESCHEDULING
To cancel or reschedule an appointment, 48 hours' notice is required. If an appointment is canceled in less than 48 hours, it will be forfeited, and the full rate will be charged. 
PAYMENT
​I require you to keep an active card on file.  I will ask for your card information in order to reserve your first session. I will not charge your card if you prefer to pay in another method such as Zelle you will have to sign a waiver. If you do not show up to an appointment, or you cancel late, I will charge your card the full session fee. Payment is due prior to the beginning of the session.
PAYMENT METHOD
I accept all major credit cards (Visa, American Express, Mastercard, Discover) through my platform in addition to a $4 processing fee, Zelle, your insurance provider, and Health Savings Accounts (HSAs) run through major credit cards.
GOOD FAITH ESTIMATE

Under Section 2799B-6 of the Public Health Service Act, healthcare providers and healthcare facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” (GFE) of expected charges. The GFE shows the costs of items and services that are reasonably expected for services provided by The Goodwell Therapy.

The estimate is based on information known at the time the estimate was created. It does not take into account any reimbursement that you may receive as a result of out-of-network benefits. If you are billed for more than this Good Faith Estimate, you have the right to dispute the bill. 

 

You can contact The GoodWell Therapy directly to address any questions or concerns pertaining to your bill. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If you are billed for more than this Good Faith Estimate, you have the right to dispute the bill.
